Affordable, Certified DOT Medical Examiners Servicing Manhattan, Brooklyn, The Bronx, Queens & Staten Island

We are NOT an Urgent Care. We SPECIALIZE in TLC / CDL / DOT Physicals. Hours: Monday-Friday 8:30AM to 4:30 PM (Extended Hours Available Upon Request) Address: 408 West 45th Street (between 9th & 10th Ave.), Ground Floor, New York, NY Drivers should bring the following items to the physical:

  • Driver’s license/photo I.D.
  • Paperwork from treating provider regarding any existing medical conditions, i.e., cardiac issues, diabetes, sleep apnea, recent surgeries
  • List of current medications, including dosage, length of time taken
  • Eyeglasses/contacts, if applicable
  • Hearing aid, if applicable
  • Current or expired medical card for re-certification
  • Any waivers or exemptions for medical reasons
  • For CPAP users, compliance of use will be needed

You do not need to bring the Medical Examination Report Form. All reports are submitted electronically in our office.

The TLC / CDL / DOT Physical, which should take approximately 20 minutes, includes:

  • Health History* (medications, recent illness/injury, high blood pressure, etc.)
  • Hearing Test
  • Vision Test (for near and farsightedness, peripheral vision, range of motion, colorblindness)
  • Urine Test (this is not a drug test; it is to test for blood, sugar, protein & specific gravity)
  • Blood Pressure Screening
  • Physical Examination

If certified, drivers will walk out with the DOT Medical Examiner’s Certificate (medical card) at the end of the physical. Once the medical examiner determines that the driver is medically fit, the driver will be certified for a maximum of two years. Drivers who take medication for blood pressure and/or diabetes will typically be certified for one year. Exemptions may be required even if a driver is deemed medically fit. For example, a FMCSA exemption is needed for drivers who are on insulin or who have acceptable vision in only one eye. Drivers who are missing a limb, or who have an impaired limb, must apply for a Skill Performance Evaluation Certificate.
